Section 59(b) of the Community Schemes Ombud Service Act requires that every community scheme, in each calendar year and at such times as may be prescribed, files the following documentation with the CSOS:

(i) an annual return in the prescribed form;
(ii) a copy of its annual financial statements; and
(iii) any other prescribed document or information.

In order for your scheme to remain compliant with this requirement of the Act, follow the steps below:

1. Download the fillable form using the link below
2. Fill in the form with the details of your scheme
3. Completed forms must be emailed to annualreturn@csos.org.za or submitted at your nearest CSOS office, along with:
   – Signed audited annual financial statement of the scheme
   – Minutes of the most recent Annual General Meeting
   – Up-to-date levy schedule
   – Details of Executive Committee (including names, ID numbers, and contact details)
   – All documentation relating to the Rules, Resolutions and Constitution of the Scheme.
   – Copy of registration certificate with CIPC/Copy of the title Deeds.
